We are a small, close knit, 3,800 patient semi-rural GP training practice that strives to maintain the very highest standards of patient care. We adhere to the traditional values of General Practice while adapting to meet the changing needs of our population.
About the Role:
We are looking for an experienced and highly skilled Advanced Nurse Practitioner (ANP) to join our team. The successful candidate will provide expert clinical care, independently assess, diagnose, and manage patients, and play a vital role in improving healthcare services within the practice. The post should ideally cover 3 working days per week including a Monday.
Key Responsibilities:
• Conduct autonomous consultations, including assessment, diagnosis, and management of patients with acute and chronic conditions.
• Prescribe medications safely and appropriately within the scope of practice as an Independent Prescriber.
• Provide evidence-based care, ensuring high clinical standards and adherence to national guidelines.
• Perform clinical examinations and request/interpreting diagnostic tests, such as blood work and imaging.
• Develop and implement personalized care plans for patients, including referrals to secondary care or specialist services.
• Support and contribute to chronic disease management, such as diabetes, asthma, and hypertension.
• Work collaboratively with GPs, nurses, and other healthcare professionals to enhance multidisciplinary patient care.
• Participate in clinical audits, service improvements, and governance initiatives.
• Provide mentorship and training to junior clinicians, including nurses and healthcare assistants.
Person Specification:
Essential:
• Registered Nurse with NMC registration.
• Master’s degree or equivalent qualification in Advanced Clinical Practice.
• Independent Prescriber qualification.
• Extensive experience in primary care, urgent care, or community settings.
• Excellent clinical decision-making skills with the ability to work independently.
• Strong communication and leadership skills to contribute to a multidisciplinary team.
Desirable:
• Experience using GP clinical systems e.g. EMIS,Docman.
• Knowledge of enhanced services, and NHS frameworks in primary care.
• Additional specialist qualifications in chronic disease management.
